Scuba diving is becoming increasingly popular in Asia, with destinations like Indonesia, the Philippines and Thailand drawing increasing numbers of enthusiasts according to a report by Vision Research Reports. Globally, the report projects the diving tourism market to reach a value of over ten billion dollars in 2032, which would be 250% increase compared to 2022.

Bali, Indonesia was the most popular dive destination on Agoda in the first half of the year. The island isn’t just a premier diving destination, it’s also known for its stunning beaches and rich culture. The island’s underwater world is teeming with marine life, making it a must-visit for divers of all levels. One of the most famous diving sites here is the USAT Liberty Shipwreck in Tulamben. This site offers divers the chance to explore a sunken World War II cargo ship and is now home to a variety of marine species.

Phuket, Thailand came in second. 🍨 Cut back on sugar

The next time you’re heaping sugar into your morning coffee, consider this: Research from the University of California, San Francisco, has found that reducing your daily sugar intake by 10 grams — roughly just over two teaspoons — could turn back your biological clock by 2.4 months. For context, a can of soda contains about 39 grams of sugar, and a chocolate bar has about 25 grams (which means eating a few squares for dessert instead of the whole thing is a step in the right direction). — E.D.

Research presented at the 2024 Alzheimer’s Association International Conference has found that eating processed meat ages the brain, contributing to a higher risk of dementia. Chan School of Public Health, replacing one daily serving of processed meat (which amounts to two slices of bacon, one hot dog, two small sausage links or two ounces of deli meat) with tofu, beans or nuts reduced a person’s dementia risk by 20%. As part of a months-long investigation, the U.S. Food and Drug Administration has issued a new alert warning about elevated lead levels in a number of ground cinnamon products. SINGAPORE, Aug. 3, 2024 /PRNewswire/ — Hepatocellular carcinoma (HCC) is one of the most lethal cancers, but Mr Ang Chin Chay, 70, was in the dark about his dire condition, until he experienced a bout of fever following a vacation to Vietnam. He quickly sought medical attention for what turned out to be a Hepatitis A infection, transmitted through contaminated food he had consumed overseas.

It was a fortuitous encounter, as routine tests revealed abnormal liver function results. Mr Ang subsequently underwent scans, which uncovered HCC, a largely asymptomatic cancer in its early stages. This was unexpected as Mr Ang was known to have fatty liver, but not cirrhosis, the most advanced form of liver scarring.

“Patients with fatty liver – but without cirrhosis – are not routinely recommended to undergo liver cancer screening,” Dr Huang explained. “But in reality, we have discovered that nearly 40 percent of HCC cases related to fatty liver occur in the absence of cirrhosis.”

Mr Ang’s prognosis would have been poor if his condition had not been detected early, as patients who are diagnosed at a late stage have an overall five-year survival rate of less than 20 percent.

Improving the screening process for patients like Mr Ang through non-invasive tests and advanced imaging is the crux of Dr Huang’s work that he will be presenting at the NUHS Scientific & Innovation Summit, on 3 August 2024.

“We aim to risk stratify people with fatty liver into those who have a high risk of liver cancer and those who do not, and to pre-emptively develop better ways to identify these people early so that they can receive some form of curative treatment,” Dr. Huang explained.

Personalising care with the digital twin

Another project pushing the frontiers of medicine with digital innovations, is the Digital Twin programme led by Dr Gao Yujia, Assistant Group Chief Technology Officer, National University Health System.

The digital twin, which is based on imaging and other data from a patient’s own tests, is a virtual 3D anatomical replica of a patient’s organs, like the liver. The digital twin will allow clinicians to transform what is usually a static scan image (such as an MRI scan), into a dynamic model that precisely recreates the shape and structural behaviour of the organ it replicates, enabling enhanced pre-surgery planning and more accurate intra-operative navigation.

Together with Holomedicine, which is a mixed reality (MR) based technology, this represents a significant leap forward in the surgeons’ ability to predict how organs will behave during a surgery, and predict how internal structures will move and change as the surgery is being performed. 

“Holomedicine leverages mixed reality to interact with virtual objects superimposed onto the real world, allowing for unparalleled accuracy in pre-surgical planning,” explained Dr Gao, who will be presenting his work in mixed reality technology at the NUHS Scientific & Innovation Summit.

With this near-pinpoint precision, surgeons can minimise the risk of injuring other structures, thereby reducing potential complications and operating time, ultimately improving the patient’s overall recovery outcomes.

Tackling diseases with precision

Dr Anand Jeyasekharan, a senior consultant at the Department of Haematology-Oncology in the National University Cancer Institute, Singapore (NCIS), likens the human body’s reaction to cancer cells, to an intense battle between two warring armies.

It was a battle that became personal in 2023 for Mr Soegianto Nagaria, Dr Jeyasekharan’s patient. He had a large tumour in his liver excised by surgeons in Taiwan. Initially thought to be liver cancer, the reports indicated it to be a Stage 4 diffuse large B-cell lymphoma instead. The situation worsened when the doctors discovered the presence of a second lymphoma in his bone marrow.

Diagnosing and treating two concurrent cancers presented significant challenges, requiring precise and coordinated medical intervention. It was important to know if the two cancers were related, as the treatment would be very different if they were.

Upon recommendations by his cousins, who are doctors, Mr Nagaria decided to continue the rest of his treatment at NCIS, under Dr Jeyasekharan’s care.

Mr Nagaria flew to Singapore in September 2023, and underwent a molecular profiling under an NUHS programme for precision oncology (IMAC – integrated molecular analysis of cancer) to get his tumour sequenced. Following the sequencing, the NCIS team discovered that the two lymphomas were not related and drew up the vital treatment plans, providing life-saving ammunition in the cancer battle.

Mr Nagaria completed multiple rounds of chemotherapy over six months, and is now on close monitoring while in remission.

Besides sequencing, which Mr Nagaria underwent, Dr Jeyasekharan is developing a new tool in his arsenal of weapons against deadly cancers – spatial phenotyping, where advanced imaging equipment is used to analyse cancer with high molecular detail.

Spatial phenotyping represents the next frontier of cancer research, and is especially useful for solid tumours. “We are trying to draw [battle] maps of cancer at a high resolution and in great detail, in order to understand how to fight it,” Dr Jeyasekharan explained. “Spatial phentoyping’s ability to dive deep into molecular detail allows us to be more precise in diagnostics, treatment and care.”

With cancer being a leading cause of death globally, spatial phenotyping can mean a world of difference for patients afflicted with cancer. “We are never happy with just delivering standard treatment, because standard treatment simply isn’t good enough,” Dr Jeyasekharan said. “Some people get cured, some people don’t – we are trying to understand what we can do to improve the chances of curing more people.”

For A/Prof Su Xinyi, who is a senior consultant with the Department of Ophthalmology at NUH, precision medicine may be the key to restoring the vision of patients suffering from currently untreatable and blinding retinal diseases. “Every patient with retinal disease has a unique gene mutation, so the treatment that we provide is tailored specifically to the mutations that they have,” A/Prof Su explained.

Besides gene therapy, A/Prof Su also explores innovations in cell therapy, which are last options for patients with end-stage age-related macular degeneration (AMD) – the third leading cause of blindness globally. In AMD, retinal pigment epithelium (RPE) cells in the retina degenerate with age. By growing these cells in a petri dish, and then injecting them into the eye, they can replace lost cells and help patients regain vision.

While this method of treatment is not new, what makes it unique is the source of cells A/Prof Su uses in its development – cells derived from the umbilical cord. By doing so, this greatly reduces the chance of rejection or the need to use immunosuppressants.

Predicting risk factors, a step ahead

Predictive care affords patients and clinicians the precious gift of time, in preventing the onset, or minimising the severity, of diseases. For Dr Jocelyn Chew, who is an assistant professor at the NUS Alice Lee Centre for Nursing Studies and Yong Loo Lin School of Medicine (NUS Medicine), adopting a predictive approach to tackling a person’s dietary triggers can curb overeating and obesity.

Dr Chew has been working on an app called the Eating Trigger-Response Inhibition Program (eTRIP), which is designed to help patients identify moments where they are prone to overeating. These include emotional eating, or even ingrained habits such as over-ordering during family gatherings.

eTRIP will be used in tandem with Adipoview, an AI-based dashboard that Dr Chew has created with the team from the NUHS Group Chief Technology Office. Adipoview will gather patient data throughout NUHS to generate insights, such as the diagnosis rate of obesity and treatment rate, to proactively predict who may need further support.

Do women need more sleep than men?

Women and men love to debate the differences between the sexes. But here’s the reality: Science confirms that women not only need more quality sleep than their male counterparts, but they also have a more challenging time getting it. Societal expectations and lifestyle differences play a role in these discrepancies, but it’s basic body chemistry that’s the real issue.

Although age and stage of life significantly influence sleep changes for both sexes, studies suggest that female sex hormones affect sleep quality and regulation. While these hormones appear to promote better sleep overall, the constant fluctuations with menstrual cycles, pregnancy, postpartum and menopause lead to disruptions in female sleep patterns and needs — a problem that most biological males don’t have to face.

Here you’ll discover how hormones influence sleep cycles, what this means for different stages of life and practical tips on how to improve sleep — regardless of sex.

Sleep requirements evolve throughout your lifetime, so how do you know how much sleep you really need? According to the Centers for Disease Control and Prevention (CDC), adults need 7-9 hours of sleep each night. That said, because sleep needs vary from person to person, you may want to think about your sleep habits toward the end of a relaxing vacation. How long do you tend to naturally sleep without the pressure of work schedules or alarms?

“There are individuals who need much less than the general average, as well as those that need more,” says sleep expert Jessica Mong, professor of pharmacology at the University of Maryland School of Medicine. She added that it’s “also dependent on one’s homeostatic sleep need or sleep debt.”

She explains that our society is chronically sleep-deprived, meaning that more sleep is usually needed for someone to feel truly rested.

An emerging area of sleep research is the role of sex differences, especially the impact of sex hormones on sleep. One novel study performed in 2013 found that women sleep about 11-13 minutes more per night than men, but there has been little research to explain why.

Sex and gender are known to affect a wide range of clinical conditions, yet not much attention has focused specifically on the link between sex and sleep differences. In a large 2020 review study, however, researchers noted the differences in sleep time between boys and girls began as young as six months and continued through childhood and adolescence. In the same review, it was found that in animal studies, oestradiol, the primary form of estrogen, significantly influences sleep and wake cycles while testosterone, the primary male reproductive hormone, seems to have no notable effect on sleep patterns.

Women’s longer sleep periods and the role estrogen appears to play on sleep cycles indicates that women likely sleep better than men thanks to their intrinsic hormones. However, that’s not necessarily the case. “While women may be sleeping an average of 10 minutes more, the duration of sleep is not as important as the timing and quality of sleep,” Mong says. “There is a clear sex difference in the circadian timing of melatonin and body temperature, with women having their peak almost an hour earlier than men.”

Mong explains that this indicates women should go to bed earlier than men, which often isn’t the case. “As a result, women may experience poor sleep quality due to this misalignment [of sleep timing], which could shift their sleep duration to be longer,” she explains.

The role of sex hormones in sleep quality and duration is most pronounced during phases of life characterized by significant hormonal shifts. Puberty marks a time of substantial hormonal changes for everyone, but women face additional challenges during pregnancy and menopause. These life phases see the most significant fluctuations in estrogen and progesterone levels, which also affect sleep.

Mong says that the influence of hormones on sleep cycles is particularly evident during pregnancy, with progesterone levels peaking at the end of the first and the beginning of the second trimester. Higher progesterone levels are associated with increased drowsiness and may even induce sleep. So while women may be sleeping more during pregnancy, it’s likely because they need it, and their hormones are helping stimulate the need.

Beyond the major hormonal shifts of puberty, pregnancy and menopause, women also experience more routine hormonal changes during their menstrual cycle. Data shows that women report higher rates of sleep disturbances and poorer quality sleep in the week before their periods — exactly when levels of estrogen and progesterone drop significantly, taking away the seemingly protective effect that these hormones have on sleep.

Sleep problems are known to increase with age across both sexes, but in women, these changes are often driven by hormonal fluctuations, societal roles and expectations.

For instance, women reach puberty earlier and typically sleep longer than men until they are 50-60 years old, the period when menopause sets in and hormone levels change dramatically. A notable difference is the elevated risk of insomnia among women of this age. “The elevated occurrence typically happens during ovarian hormone fluctuations, particularly around perimenopause and into menopause,” Mong says. Even so, if you are experiencing these menopause-related challenges, there are treatment options that may be helpful, including cognitive behavioral therapy and hormone therapy. “Estrogens have been shown to be an effective treatment for sleep disruption during this time,” Mong says.

Finally, post-menopause, women experience continued alterations in core body temperature, melatonin rhythms and circadian cycles, leading to reduced sleep duration and quality.

One other factor that’s impossible to ignore is the role of parenthood and caregiving on women’s sleep needs and quality. While women may tend to sleep slightly more than men, work schedules and family responsibilities significantly affect the quality of their sleep. For instance, one 2023 study found that while new dads of infants slept less than new moms, the new moms had poorer sleep quality than their male partners.

Numerous holistic methods as well as over-the-counter and prescription treatments can help you achieve a better night’s sleep. While hormone-driven changes are beyond your control, you can take steps to enhance your sleep quality.

Mong believes that sleep deprivation in our society stems from several factors. “Electronic devices and the ability for us to continue activity after dark are major contributors to chronic sleep deprivation,” she explains. “Other contributors include shift work, circadian timing disorders and neurodegenerative diseases.”

To improve sleep hygiene, Mong recommends shutting down electronic devices well before bedtime. “Power down your television, phone or other devices two hours before bedtime to ensure you get between 7 and 8 hours of sleep,” she suggests. “This means turning off devices between 9 and 11 hours before you need to wake up. Do this for a week and see if you wake up feeling more rested.

It’s generally best to allow your natural sleep cycle to dictate the duration of sleep rather than your alarm clock.

Adults should get 7-9 hours of sleep a night. There’s no sex-specific recommendation, but research suggests that women sleep about 11 minutes more than men, on average.

While there’s no precise number, the menstrual cycle frequently causes sleep disruptions. Most women notice the most significant sleep disturbances in the week before their period. It’s likely worthwhile to prioritize sleep during this week to help offset any sleep disruptions you experience.

All adults need 7-9 hours of sleep per night, although this varies from person to person.

Delta CEO offers employees free flights after CrowdStrike-Microsoft chaos

Delta Air Lines CEO Ed Bastian on Friday offered employees two free travel passes to thank staff members who were caught in massive disruptions last month sparked by a botched CrowdStrike software update that stranded thousands of customers and crew.

Delta had more trouble than competitors in recovering from the outages that took thousands of Windows machines offline around the world, affecting industries from health care to banking.

The carrier canceled more than 5,000 flights between July 19 and July 24, more than it did in all of 2019, according to FlightAware. Bastian said earlier this week that the incident cost the company about $500 million, a sum that is equal to about 40% of Delta’s second-quarter profit. A crew-tracking platform was a contributor to the cancellations and disruptions, the airline has said.

Delta told CNBC’s “Squawk Box” on Wednesday that the airline had to manually reset 40,000 servers.

The disruption “has been a humbling moment for our company,” Bastian said in his note on Friday, which was seen by CNBC. “I know it’s been extremely difficult, and I’m deeply sorry for what you have endured. An operational disruption of this length and magnitude is simply unacceptable — you and our customers deserve better.”

Upward of 4,000 Delta flight attendants picked up more than 6,100 trips during the disruptions, receiving extra pay, according to another Delta staff memo on Friday.

“Your efforts throughout have been nothing short of heroic,” Bastian told staff.

The two “positive space” passes Bastian offered employees are confirmed seats like a customer would have, different from the free standby flying airline employees often do if there are available seats.

The Delta organizing committee of the Association of Flight Attendants-CWA, which is in the middle of a campaign to unionize Delta’s flight attendants, said the offer of passes “just isn’t going to cut it.” The organizing committee, in a written statement, said airline management routinely makes “meager adjustments to keep the operation running without making changes significant enough to prevent a future meltdown.”

Delta’s operation has since stabilized but the flight cancellations and delays stranded thousands and scarred Delta’s high reliability standings. Its executives frequently point out Delta’s successful work to win over both leisure and corporate customers who are willing to pay more to fly the carrier, marketing itself as a premium airline.

A Delta spokesman earlier this week said the airline has processed “thousands” of refunds and reimbursement requests. 

The U.S. Department of Transportation is investigating Delta’s disruptions, Transportation Secretary Pete Buttigieg said last week. Similar disruptions at other carriers, such as the massive 2022 holiday meltdown at Southwest Airlines after winter storms, have highlighted how technology issues can severely disrupt air travel.

Bastian said Delta plans to pursue legal action against CrowdStrike and Microsoft “to recover our losses caused by the outage” and that it has hired law firm Boies Schiller Flexner.

Microsoft declined to comment. CrowdStrike said it has “no knowledge of a lawsuit and have no further comment.”
